Our clinic will be open Tuesday and Thursday from 10:00 AM to 2:00 PM. We greatly encourage all booster recipients to bring their vaccine card so accurate documentation may be kept of their vaccine history. Our Vaccine Hotline can still be reached at

Evans Memorial is closely monitoring and following guidelines set by the CDC and FDA.  Evans Memorial continues to stay close to the situation and will update the community as new guidelines are given.
